What is MyChart?

MyChart is a free, internet-based service that allows patients to access personal medical information online. With MyChart you have access to your personal health information from any computer or mobile device at any time.

Access MyChart

Our MyChart patient portal contains your medical records beginning on February 25, 2017. Past medical records can be obtained by calling Health Information Management at 715-532-5561, ext. 41217. To access our previous patient portal, click here.

What information is available in MyChart?

Use MyChart to:

  • refill and track prescriptions
  • view your immunization records
  • view your list of allergies
  • view lab and test results as soon as they’re available
  • review your health record to understand your personal wellness
  • click on your current health issues, allergies or medications to learn more about each topic from the National Library of Medicine.


Because life can be a whirlwind of activity, MyChart lets you send secure messages to  your provider’s office whenever your schedule allows — without playing phone tag.

  • request an appointment with a provider you have seen
  • schedule appointments online with certain providers
  • check the time of and confirm future appointments
  • get directions to an upcoming appointment

Proxy Access:

My Family’s Records
Are you the medical decision maker in your family? Do you care for an elderly relative? With proper consent, you may use features of MyChart to manage the health information of your spouse, children and elderly relative(s). For more information on proxy access, click here.


  • Review the insurance information on file at Marshfield Medical Center - Ladysmith for your account.
  • Add, delete or edit your insurance information to save time at your next appointment.


You can go online to read a copy of any letter sent by your provider's office for up to two years.If you need a complete copy of your medical record, please contact our Health Information Management offices for information about the Release of Information process. They can be reached by calling 715-532-5561 ext. 41251.


Under Download Summary, you’ll find your Lucy Record – a portable copy of your allergies, medications, current health issues, procedures, test results and immunizations from MyChart. If you’re traveling outside of our area, download your Lucy Record to a USB drive and bring it with you so your information is readily available if you need it.
