Concerns & Complaints

We appreciate your feedback and want to ensure we do what we can to resolve any issues.

A patient, sponsor, family member, visitor or advocate may file a verbal or written complaint regarding treatment, abuse, neglect, harassment, medical care, theft of property or behavior of other residents or staff members without fear or concern.

Please contact us with any concerns you have regarding your care. We encourage you to let us know by sharing your concern with staff directly, contacting our Quality Director at 715-532-5561 ext. 1364, or filing a written complaint.

When filing a written complaint, follow this procedure:

  2. Answer all questions on the form accurately.
  3. Sign and date the form.
  4. Give the completed report form to the front registration desk at the hospital or mail to:

Our staff is here to ensure your complaints are taken seriously and without discrimination.
