Concerns & Complaints

We appreciate your feedback and want to ensure we do what we can to resolve any issues.

A patient, sponsor, family member, visitor or advocate may file a verbal or written complaint regarding treatment, abuse, neglect, harassment, medical care, theft of property or behavior of other residents or staff members without fear or concern.

Please contact us with any concerns you have regarding your care. We encourage you to let us know by sharing your concern with staff directly, contacting our Quality Director at 715-532-5561 ext. 41364, or filing a written complaint.

When filing a written complaint, follow this procedure:

  1. Obtain a Patient Complaint Form from the nurses’ station, registration desk or business office or download here.
  2. Answer all questions on the form accurately.
  3. Sign and date the form.
  4. Give the completed report form to the front registration desk at the hospital or mail to:

Our staff is here to ensure your complaints are taken seriously and without discrimination.

(Note: Complaints of abuse, harassment, or mistreatment will be immediately investigated and you will receive an oral and written report of the findings, recommendations, and/or corrective action taken within five working days or the filing of the report.)

Marshfield Medical Center - Ladysmith offers many amenities and conveniences for visitors at our facility. Along with visitation hours, visitors also have access to our Cyber Café, chapel services, Gift Shop, cafeteria and free visitor parking.
